PASSPORT REQUIREMENTS

• Original certified Birth Certificate or original naturalization papers.
• Two (2) passport pictures (or take pictures at the show for $15).
• State issued drivers license. *State issued identification cards can be accepted
• The USPS accepts two forms of payment. Check or cash. If paying by check bring two checks one for the USPS $30 and one for the US State Department $67 for adults.
• Complete form DS-11 (passport application) available at the show.
• If applying for children 15 & under both parents must be present.
