are accepting passport  applications at our events

EXCEPT MINNEAPOLIS including taking passport photographs.

PASSPORT REQUIREMENTS

• Original certified Birth Certificate or original naturalization papers

            (no photo copies or hospital documents).

• Two (2) passport pictures (or take pictures at the show for $15.

            In Des Moines photo cost is $10.

• State issued drivers license. *State issued identification cards can be accepted.
• The USPS accepts two forms of payment. Check or cash. If paying by check bring two checks one for the USPS/Polk County and one for the

           US State Department. In Des Moines only checks are accepted

• Complete form DS-11 (passport application) available at the show.
• If applying for children 15 & under both parents must be present.
